The visa fee is paid once at the time of application. The exact amount depends on whether you apply from inside or outside the UK, the length of stay, and whether a lower fee applies.
Immigration Health Surcharge (IHS)
The Immigration Health Surcharge gives you access to the UK's National Health Service (NHS) during your stay. Most applicants pay it. Health and Care Worker visa applicants do not, and nor do people applying from outside the UK for a stay of 6 months or less (GOV.UK).
