Summary of duties include: - Working on the referrals received via the CRS Refer and Monitor system. - Develop good and effective working relationships with Local Authority housing teams and other accommodation providers. - Work with and support individuals placed in Approved Premises and CAS2 for move on accommodation. - Work alongside fellow HPT colleagues who work on the CAS3 provision to have access to the CAS3 booking system where other options have been explored. - Represent the team at local housing forums and at prison leaver meetings. - Attend CAS3 move-on meetings, supporting the SPO chair in gaining updates from PPs and external partners in preparing for the meetings and having solution focussed discussions. - The base location is optional but there is a requirement to travel to other offices list based on operational requirement.
